The Advantages of a Deep Plane Facelift
The goal of a deep plane facelift is to lift the tissues of the face by going under the superficial muscular aponeurotic system, the SMAS, which is the investing layer of the face. This gives the sturdiest lift and, in Dr. Sykes’ opinion, the most natural lift. He lifts the skin on the neck and tightens it to create a better angle to the chin and neckline. By lifting the jowls so that they disappear, the mandible – or jawline – achieves better definition.
Dr. Sykes also lifts the cheeks to make the face appear top-heavy rather than bottom-heavy. A youthful face is shaped like an upside-down triangle, but as we age and the cheek fat and jowls descend, the triangle becomes upright with more volume at the bottom. When we lift those areas into a better position, it improves and increases the volume of the face in the right places. Patients who have scheduled a consultation for lower facelifting should carefully research their surgeon and be aware of their specific desires regarding the outcome of surgery. Each patient should consider their wants and needs before the consultation to enhance their communication during the consultation.
Before surgery, it is important to thoroughly read all instructions, including understanding and carrying out the things to avoid. These include avoiding sun exposure a few weeks before surgery, stopping alcohol intake one week before surgery, and avoiding smoking for at least two weeks. The length of time varies from surgeon to surgeon related to their surgical skill and experience. However, in our practice, a facelift and neck lift on most patients takes significantly less surgical time—1 to 1 and a half hours. In our practice, these lower operative times diminish the incidents of hematoma and other complications and require less anesthetic time for the patient.
The age range for patients is vast, starting in the mid-30s up until the mid-80s. However, the most popular age range for a facelift is 45 to 65.
A mini facelift requires shorter scars and less operative dissection. However, this procedure allows much less correction of aging problems. For this reason, Dr. Sykes rarely performs it. Many patients ask for a mini facelift, but they do not want mini-facelift results—they often want the full rejuvenation of a traditional facelift.
In Dr. Sykes’ opinion, there are no acceptable alternatives to create the results that a surgical facelift creates. Non-surgical options, such as a liquid facelift and PDO threadlift, can provide minor facial improvements. The effects these treatments produce are minimal compared to a deep facelift.
To extend facelift results, it is important to avoid sun exposure, use routine skincare, visit an esthetician regularly, and have minimally invasive procedures, such as Botox®, facial fillers, and laser procedures, to improve the signs of aging.
Most patients can go out after a surgical facelift 7 to 10 days after the procedure. However, if a patient wants to look great, especially in most photographs, that may take 3 to 4 weeks.
Dr. Sykes likes patients to take at least 3 weeks off from exercising after all surgical procedures, including a facelift. During follow-up visits, Dr. Sykes will advise when it is safe to resume normal workouts.
The length of time off after a facelift depends entirely on what type of work the patient does. If the patient works at home and does computer work, they can likely work 3 to 4 days after the procedure. However, if the patient has a job that requires being out in public, such as sales, they may need 10 days before returning to work.
Many surgeons will not perform facelifts on patients who actively smoke. The reasons for this are that smoking diminishes blood supply and increases the risk of skin compromise and post-operative complications. Dr. Sykes has performed facelifts on smokers with very good results, but we have slightly altered the length of the skin flap to diminish the risk of vascular compromise to the tissue. This alternation and technique have worked very well in Dr. Sykes’ practice.
Like any surgery, a facelift comes with risks. These risks may include post-operative bleeding, infection, skin flap compromise, or motor nerve injury. In our practice, the risk of hematoma is very low. Dr. Sykes has never had a patient experience motor nerve injury after facelift procedures.
Another risk after a facelift is poor scarring. However, we have many options today that alter and improve the scars after a facelift. These include perioperative hyperbaric oxygen treatments, steroid injections into hypertrophic scars, and advanced post-operative laser treatments that radically improve scar appearance.
